Sports Physical Requirements:
Please turn in all required medical information to the office if your child will be trying out for any sport this 2008-2009 school year.
A new sports packet physical is required for each school year. This has your signature along with your child's signature. It must also be signed and dated by your child's physician.
An EKG or Echocardiogram is also required for each new school year.
Students will not be allowed to try-out for any sport without the required forms. There are no exceptions!
