New "OK to Play" Program for Extracurricular Sports

All students who wish to try out for a Lincoln Hall sports team or play in school intamurals will need to pick up an " OK to Play" green slip in the health office.

The "OK to Play" slip notifies the coach that all necessary information is in the health office. This includes:

1. A current 2009 sports physical or Illinois State Health Examination form;

2. A copy of  the 2009 EKG/Echocardiogram signed by your physician or a copy of the letter you received from One Heart Champions (If you participated in the echocardiogram program at Lincoln Hall).

Sports Physical Requirements

Sports Physical Requirements:

Please turn in all required medical information to the office if your child will be trying out for any sport this 2009-2010 school year.

A new Sports Physical form is required for each school year. It must also be signed and dated by your child's physician.

An EKG or Echocardiogram is also required for each new school year.

Students will not be allowed to try-out for any sport without the required forms.  There are no exceptions!

Strep Throat- Streptococcal Infection

Please watch for the following signs and symptoms:

* Sudden onset of fever with sore throat pain.
* Discomfort with swallowing and tender, swollen glands in the neck.
* Some children report having a stomach ache and/or headache.

Diagnosis can only be made with a throat culture performed by your physician. Please wait until you receive the results of the culture before sending your child back to school. Strep throat, which includes a fine rash, is called Scarlet Fever. Treatment is the same for both forms of Strep.

Symptoms usually present one to four days after exposure. Transmittion is usually upper respiratory. Treatment includes antibiotics therapy by prescription.

Your child can return to school after a full 24 hours on antibiotic therapy and when he/she is fever free for 24 hours. A physicians note is also required to return to school.
