Summer School Program 2010
June 21 - July 16, 2010

SCHOOL DISTRICT 74 REGISTRATION FORM ONLY (to print, fill out and mail or drop off to Lincolnwood School District - see instructions on form).

INFORMATION:

Classes are held on the Lincolnwood School District 74 campus at: Lincoln Hall Middle School, 6855 N. Crawford Ave., Lincolnwood, IL 60712
Student placement is determined by his/her grade for the upcoming 2010-2011 school year
There are 4 class sessions daily; 8:30 am, 9:30 am, 10:30 am, and 11:30 am
Each class is 55 minutes long
Students may enroll in 1, 2, 3 or 4 classes
Tuition is $90.00 per class hour. Some classes may have additional material fees
The program is open to all students entering Kindergarten through 8th grade. There is no extra fee for out of district students
Parents are responsible for transportation to and from summer school. Bus service is not available except for students being transported to Camp 74 after the 11:30 session

Registration:

You may register by doing one of the following. Please complete one form per child.

2) In Person: Drop off completed form(s) at: Lincolnwood District Office 6950 N. East Prairie Road Lincolnwood, IL 60712

Include check made payable to Lincolnwood School District 74 or you may pay by credit card or E-Check online at www.sd74.org (click E-Pay)

Classes are filled on a first come/first served basis. Registration is not complete until both registration form and payment have been received. Classes with insufficient registration may be cancelled. Tuitions are non-refundable except for: medical reason, exclusion from class due to class limit being met or inappropriate class registration. No refunds after June 21st.

Late Registration June 5-18, a $25 late fee, per student, applies
(Note: There will be NO registration after June 18)
Open House Friday, June 18, 10:30-12:00
1st Day of Classes Monday, June 21, 2010

Last Day of Classes Friday, July 16, 2010
